The USS Hiawatha (NCC-815) was a Starfleet medical frigate in service in the mid-23rd century. The ship was damaged and subsequently crashed onto an asteroid after encountering fluctuating gravity waves during the Federation-Klingon war of 2256-57.
Her wreck was later found by the USS Discovery (NCC-1031) ten months after the crash.
