This is the Klingon Vor'cha Class with an alternate front module, based on a design sketch from Rick Sternbach he made for the Vor'cha. He envisioned the head to be an interchangeable, mission specific module, that can also act as a life boat in emergency situations. Sadly, this idea was never realized and we only saw the Vor'cha on screen just with the Disruptor Module. This is the Command Module, as it features a command tower, reminiscent of the old D7/K'Tinga model. Maybe this is the look of the original Vor'cha Class prototype, the IKS Vor'cha.
