This is the Saladin/Hermes Class from the Star Trek Star Fleet Technical Manual by Franz Joseph Schnaubelt. Although the book is not considered canon, the page with this ship was used as a computer display on Star Trek II. This model depicts the Saladin Class Destroyer. To make this a Hermes Class Scout, remove the two phaser emitter on each side of the upper saucer, as also the two photon torpedo launcher on the base of the bridge dome with a small file or fine sand paper.
